The light source is not correctly installed.

Check if the light source is properly installed. Ensure that the fixation bolts of the diffuser are properly mounted onto the light unit.

Safety switch damaged

Check Light Source Safety switches for damage. Replace if damaged.

Safety switch disconnected

Check connection of safety switch at the LCB. If connections are OK, then measure switch cable for continuity. Replace the cable if necessary.
